Cancer chemotherapy, especially alkylating agents, increases the risk of leukemia and other secondary cancers. Immunosuppressive agents increase the risk of some cancers, especially lymphoma.44 While hormones and hormone antagonists increase the risk of some cancers, they decrease it for others. Hormone replacement therapy with estrogens for the treatment of menopausal symptoms increases the risk of endometrial cancer and breast cancer.34 Oral contraceptives reduce by half the risk of ovarian and endometrial cancers; but they may increase the risk of liver cancer.34 It should be emphasized that overall the risk of cancer associated with conventional medicines is very much smaller than the direct benefits of therapy.

Although changes in lifestyle can reduce cancer incidence, such changes may be very difficult to achieve, as anyone who has tried to stop smoking can attest. As the last two decades have brought little gain in terms of survival benefit to patients with most types of cancer, there is now a major research emphasis on the application of behavioral science in health promotion and prevention programs to create lifestyle change at the population level that reduce the incidence of the disease in the population.
