Introduction

Cancer remains one of the major causes of death in the world and it has been estimated that there will be 15 million new cases and 10 million deaths in 2020. Since mustine was first used to treat an acute lymphoblastic leukemia patient in 1943,2 approximately 100 cancer drugs (and counting) have been approved by the US Food and Drug Administration (FDA). During the last 63 years, survival in some cancers has improved but the urgent need to develop new and improved drugs has continued. Initially, therapies were directly cytotoxic, consisting of alkylating agents, antimetabolites, and antitumor antibiotics. As biological understanding of cancer cell growth kinetics continued to increase, other successful strategies were employed, i.e., interruption of tubulin/microtubule formation, disruption of normal nucleotide incorporation, interference with nuclear hormone receptors or inhibition of topoisomerases I and II,3 thereby increasing the cytotoxic arsenal for killing or slowing the growth of tumor cells. While these drugs continue to be refined, resistance reduced, or drugs combined to improve survival and/or reduce toxicities in cancer patients, novel molecular targets continue to emerge as additional opportunities to increase effectiveness of standard therapeutics.

The important characteristics differentiating cancer cells from normal cells are: (1) limitless replicative potential; (2) self-sufficiency in growth signals; (3) nonresponsiveness to normal growth-inhibitory signals; (4) escape from senescence and apoptotic death; (5) ability to invade normal tissues and metastasize to distant sites; and (6) support angiogenesis within the growing tumor - all characteristics targeted by recent therapies. In addition, the modulation of resistance with new drugs to potentate existing chemotherapeutic agents and/or the chemoprevention of primary tumors provide exciting new targets for cancer therapeutics.
