Fluid Balance

Several factors lead to decreased fluid loss or ambiguity of volume status during laparoscopy.

• Evaporative fluid loss normally seen with open procedures is negligible. In addition, insufflated air is usually humidified.

• Increased intraabdominal pressure results in decreased renal blood flow with resultant decrease in GFR and urine output. This is aggravated by a secondary release of renin and ADH resulting in further reabsorption of sodium and free water leading to anuria.

The transient decrease in intraoperative blood flow however, does not seem to produce significant renal dysfunction postoperatively.
