Gastroesophageal Reflux

Gastroesophageal reflux should be suspected if there is frequent emesis and obstruction has been ruled out. Its workup involves barium swallow and 24 hour pH probe. Initial treatment should involve thickening the formula with rice cereal and keeping the patient upright while eating. Medicinal management includes H2 blockers and prokinetic agents. Surgical therapy is reserved for those in which conservative measures prove inadequate or who have complications such as stricture formation or bleeding. Surgery involves the Nissen or Thal fundoplication.
