Meconium Ileus

Obstruction can be caused by meconium impaction within the ileum. It is most commonly encountered in newborns with cystic fibrosis. Workup involves a contrast enema which may reveal a microcolon and meconium within the distal ileum. If there are no complicating problems, it is appropriate to attempt conservative therapy first, which includes gastrografin enemas until the meconium passes. Surgical therapy is instituted when conservative measures fail, or there are complications such as perforation. It includes resection of the distended terminal ileum with os-tomy or end-to-end anastomosis.
