Mucoepidermoid Carcinoma

It is the most common malignant tumor of parotid and salivary glands responsible for 35% of salivary tumors. They are of mucin and epithelial cell derivation.

They are subdivided into:

• Low grade—well differentiated

• High grade—poorly differentiated

High grade has a high incidence of locoregional recurrence, distant metastasis, and shorter survival. Over 70% have lymphatic metastasis. High grade—treat with superficial/total parotidectomy with facial nerve sacrifice if necessary and if no clinical nodal disease do a selective nodal dissection. If palpable nodal disease is present, a formal neck dissection is performed.

Low grade—treat with superficial lobectomy and spare facial nerve unless directly involved; then resect and cable graft it with donor nerve.
