Chloroquine can induce hyperpigmentation, exanthema, exfoliative reactions, hyper-sensitivity to light, and pruritus.

Hyperpigmentation, that is, slate-gray to blue-black macules before the lower leg, in the face, the soft palate, or subungual are caused by hemosiderin and melanin. They disappear spontaneously within 11-20 months. In one case report, a depigmentation (such as vitiligo) was described that was reversible after discontinuation of chloroquine.

Exanthema can be maculopapular, lichenoid, or urticarial or can look like an erythema anulare centrifugum. Also, single cases with exfoliative reactions and toxic epidermal necrolyses were reported (Murphy and Carmichael 2001).

Chloroquine can induce photoallergic andphototoxic reactions. This has to be kept in mind if a light-sensitive dermatosis treated with chloroquine deteriorates (Metayer et al. 2001, Ochsendorf and Runne 1991).

Pruritus is mainly reported by black people during malaria prophylaxis and therapy (8%-74%). Within 11-48 h after intake of the drug, a generalized pruritus, especially on the palms, soles, and scalp, without visible skin changes starts. Antihist-amines were of no benefit. Recently, the pruritus was attributed to opioidergic effects in a rat model. Promethazin and corticosteroids were of therapeutic benefit (Onig-bogi 2000). Individuals with pruritus had an altered metabolism after therapeutic doses of chloroquine (Onyeji and Ogunbona 2001).

In prospective studies with large patient numbers, no exacerbation of psoriasis could be demonstrated statistically, and in some cases the psoriasis even got better. However, the psoriasis of individual patients may deteriorate under chloroquine.
