Colonic complications of graft-versus-host (GVH) disease are common. In this disorder, mature donor lymphocytes attack recipient tissue in bone marrow transplantation patients. Acute GVH disease may manifest with profuse diarrhea, nausea, vomiting, intestinal hemorrhage, and cramping abdominal pain. Transmural inflammation is common but frank perforation is unusual.

Initially, CT shows diffuse mural thickening of the colon with submucosal edema and increased intraluminal secretions. In chronic GVH disease, submucosal fat may deposit rapidly and be readily identified on CT [4].
