Kinetic Redundancy and Models of Spinal Loading

The maximum allowable muscle stress was kept constant for all the models at 55 N cm2. The predicted maximum isometric strength at upright posture for different models ranged from 54 to 163 Nm for flexion, 104 to 351 Nm for extension, 109 to 240 Nm for right lateral bending, and 57 to 266 Nm for right axial rotation. The differences between models led to substantial variation in the predicted strength and joint reaction forces. For example, in a complex loading condition of 30 Nm of flexion, right lateral and axial moments, using the second cost function, the predicted compression forces ranged from 410 N to 1153 N, while the anterior-posterior shear forces ranged from -254 N to 76 N. Generation of Muscle Founder Cells by Asymmetric Division

Clearly, however, asymmetric division by muscle progenitors is a crucial step in the generation of muscle founder cells with distinct identities. In each of the progenitor cell divisions, daughter cells acquire a different cell fate as a consequence of asymmetric distribution of molecules. Insights into the identity of these molecules and possible functions in the somatic musculature came from studies on neurogenesis. Determination and Differentiation of Visceral Myoblasts

Another gene critical to visceral muscle development is the recently identified jelly-belly (jeb) locus, which encodes a signaling protein with an LDL receptor-repeat (Weiss et al., 2001). Using a yeast assay system, jeb was isolated in a screen for genes that were regulated transcriptionally by tin. As anticipated from this method of isolation, ectopic expression of tin results in ectopic jeb, indicating that tin is sufficient for its expression. Redundant mechanisms exist for jeb activation, however, since loss of tin has no significant impact on the presence of jeb transcripts. Interestingly, jeb is actually synthesized in the somatic mesoderm. Motor Neuron Innervation

In contrast to the hypothesis put forward by these studies, however, it is clear that motoneuron innervation does not contribute to the patterning of the larval body wall muscles. Indeed, as elaborated in Section 2.1.3, the pattern of muscle fibers is dictated by the presence of single founder cells that seed the fusion process and are specified hours before innervation occurs. Broadie and Bate sought to address this issue directly, and examined muscle development in embryos in which motoneuron development is delayed (Broadie and Bate, 1993b). Their observations revealed that, in the absence of motor nerves, myoblasts can fuse normally, form proper attachments, and develop the electrical properties of larval muscles in a normal time course. Thus, innervation appears to play no role in patterning, morphogenesis, maintenance, or physiological development of the somatic muscles in the Drosophila embryo (Broadie and Bate, 1993b).

An important aspect of muscular development is the relation between nerve and muscle. Most experiments in which innervation was suppressed have shown neural dependence during the different stages of myogenesis (4-6). Information about the nature of interactions between neuroblasts and myogenic cells can be obtained through experimental alterations of the spatial relations normally existing among these cells. This procedure is easy to carry out in the avian embryo in ovo but not in the mammalian fetus in utero. However, this difficulty can be overcome through the use of a technique developed by us, which consists in implanting xenografts of fetal mouse tissue into the avian embryo. This procedure is of particular interest because it allows the murine fetal cell to be studied by the classic methods of experimental embryology in terms of what happens to the avian embryo cell.

Primary Cardiac Arrest during Exercise

Myocardial ischemia, arrhythmias, and sudden death can occur during or immediately after exercise.1 Cardiovascular function is under the regulatory control of the sympathetic nervous system, with sympathetic stimulation causing an increase in heart rate, force of myocardial contraction, and blood pressure. During exercise, there is a progressive rise in systolic pressure, though usually very little, if any, change in diastolic pressure. Diastolic pressure may fall slightly in younger individuals and rise in middle-aged and older individuals.2 Heart rate increases progressively with exercise before reaching a plateau. The average maximum heart rate is approximately 180-200 beats per minute. Systolic blood pressure increases to between 162 and 216 mm. Experimental Disease Models

The Mdr1a mouse model is one of the few genetic disease models that develop a colitic phenotype in the absence of immune dysfunction. This is in contrast to virtually all other genetically targeted mice, which develop colitis as a result of either impaired immune function, a cytokine imbalance, or colitis that can be induced by reconstituting naive (CD4 + CD45Rbhi) T cells into severe combined immunodeficiency (SCID) mice. Most of the experimental models of IBD in mice have inflammation only in the colon and resemble UC. The one striking exception to this is the SAMP1 Yit mouse and the derivative SAMP1 YitFc strain. These mice spontaneously develop a Crohn's-like transmural ileitis as early as 10 weeks of age, accompanied by prominent muscular hypertrophy, fibrosis, and activation of mesenteric lymph node lymphocytes, which produce high levels of interferon g (IFN-g). Furthermore, a subgroup of SAMP1 YitFc mice ( 5 ) also develops perianal fistulating disease.

Clinical Uses Of Electrical Stimulation

Maintenance or increase in range of movement. Electrical muscle stimulation (EMS) is used to strengthen muscle and facilitate voluntary motor function. Although EMS devices are often advertised for muscle toning and weight reduction, they are authorized by the FDA only as prescription devices for maintaining or increasing range of motion, relaxation of muscle spasm, prevention or retardation of disuse atrophy, muscle reeducation, increasing local blood circulation, and postsurgical stimulation of calf muscles to prevent the formation of blood clots.

Primary megaureter corresponds to an obstructive dilatation of the ureter above an adynamic segment at the ureterovesical junction. It is caused by hypoplasia and atrophy of muscle fibres in the distal segment of the ureter associated with an increase in collagen fibres in the pathologic segment and muscular hypertrophy of the proximal ureter.

The diagnosis of idiopathic megacolon and megarectum is a diagnosis of exclusion, made only after other causes of constipation such as Hirschsprung's disease have been ruled out (Fig. 3.6). This syndrome can be distinguished from Hirschsprung's disease by the presence of anorectal inhibitory reflex, the absence of a normal caliber or stenotic length of distal bowel, and the presence of ganglion cells on rectal biopsy. Subtle histologic abnormalities have been identified in both smooth muscle and intrinsic innervation. There is thickening of the bowel wall due to smooth muscle hypertrophy and fibrosis of the muscularis mucosa, circular muscle, and longitudinal muscle layers. These findings, however, are not specific to this syndrome.


Diverticulitis Magnetic Resonance

Using CT, diverticula are seen at the site of perforation or adjacent to it in about 80 of cases. They appear as small out-pouchings of air, contrast, or fecal material projecting through the colonic wall. Symmetric mural thickening of the involved colon by approximately 4-10 mm is seen in about 70 of cases, however if there is marked muscular hypertrophy, the wall of the colon can measure up to 2-3 cm in thickness.


A contracture is an abnormal, often permanent, condition characterized by flexion and fixation of a limb at a joint. Contractures leave the joint in a non-functional position, resistant to bending. They are caused by atrophy and abnormal shortening of muscle fibers. Their primary cause is disuse. Prolonged bed rest, even in normal individuals, results in loss of lean muscle mass through lack of use. The muscles become weak, atrophic, change shape, and shorten with disuse. The muscle decreases in diameter and in the number of muscle cells. Eventually, there may be replacement with fibrous connective tissue, progressing to fibrosis and development of contractures.

Pyloric Stenosis

Pyloric stenosis typically presents in first born males, aged 4-8 weeks, as increasingly projectile nonbilious emesis. It is caused by muscular hypertrophy of the pylorus, which results in outflow obstruction. Physical exam may reveal a classic palpable olive in the right upper quadrant or upper mid abdomen. Electrolyte abnormalities are common and include metabolic alkalosis with potassium and chloride depletion. Diagnosis can be confirmed with ultrasound or upper GI series. Treatment involves fluid resuscitation and correction of electrolyte abnormalities, followed by the Fredet-Ramstedt pyloromyotomy, which is incision and spreading open of the seromuscular layer of the pylorus. Postoperative feeding protocols involve frequent small feeds of progressively less dilute formula over approximately two days.
