Clients with chronic PTSD often face multiple life stressors that lead to impaired general functioning. In addition, individuals with chronic PTSD often have comorbid psychiatric and medical problems (e.g., Davidson, Hughes, Blazer, & George, 1991 Kessler et al., 1995). Therefore, crises during treatment are not unusual, especially if early or multiple traumatic experiences have interfered with the development of healthy coping skills. Poorly modulated affect, self-destructive impulse-control problems (e.g., alcohol binges, substance abuse, risky behaviors), numerous conflicts with family members or others, and severe depression with suicidal ideation are common comorbid conditions with PTSD. These problems require attention but can potentially disrupt the focus on treatment of PTSD. If careful pretreat-ment assessment has determined that chronic PTSD is the client's primary problem, our goal is to maintain the focus on PTSD with periodic reassessment of other problem areas, as needed.

We observe that, while using very conservative Z scores that essentially exclude false positives, the new protocol recognizes correctly (without any information about sequences) most of the family members with the RMS distance between the superimposed side chain centers of up to 4 AA. Most EV-infected neonates are thought to be infected either intrapartum by exposure to maternal blood or genital secretions or after delivery by exposure to oropharyn-geal secretions or stool of mothers or other contacts (6,34,37). EVs have been grown from cervical swabs obtained from symptomatic pregnant women and from mothers of infected, ill neonates (62,66-68). Shedding of virus from maternal throat and rectum during pregnancy has also been documented, with or without symptoms (69). A small number of victims have been chronically disabled by the earthquake. They are suffering from head injuries and spinal cord injuries, and some have developed paraplegia. A high-risk group for chronic stress disorders is elderly males who lost their family members. However, large-scale epidemiologic studies have yet to be completed for the victims of the earthquake.

Research reviewed elsewhere shows that trauma-related guilt is a common problem for survivors of many different kinds of traumatic events-including survivors of combat, physical and sexual abuse, technological disasters, and surviving family members of victims of accidents, suicide, homicide, and sudden illness (Kubany et al., 1995 Kubany & Manke, 1995). Others may become permanently dependent on family members, friends, or healthcare providers for all their daily care. Many people who do not immediately or fully recover from a stroke work with professional therapists who help them adapt to their limitations and function as independently as possible. Some people who have had a stroke may experience inconsistent and unpredictable mood changes. For example, they may laugh or cry inappropriately or may become irritable without apparent cause.
