Treatment of Eczema

Creams are suitable for moist or weeping areas of eczema, whereas ointments should be used for dry, scaly, or lichenified areas. Glycolic acid has been recognized as an important adjunctive therapy in a variety of conditions including photodamage, acne, rosacea, striae albae pseudofolliculitis barbae, hyper-pigmentation disorders, actinic keratoses, fine wrinkles, lentigines, melasma and seborrheic keratoses 5 . Moreover, it can reduce UV-in-duced skin tumor development and it has been proposed as a therapeutic modality against skin exfoliative conditions such as ichthyosis, xeroderma and psoriasis. In post-menopausal women a cream containing 0.01 estradiol and 15 glycolic acid, applied to one side of the face for 6 months, induces a significant improvement in reversing markers (rete peg pattern, epidermal thickness) of skin aging 6 .

Specific History

Striae occur in 35 to 40 of pubescent boys and 70 of pubescent girls, with a peak incidence about age 16 years. They occur more readily in patients with a history of rapid weight gain, and adolescents seem more prone to develop lesions associated with physical exercise and corticosteroid exposure. Striae gravidarum develop to some extent in up to 90 of pregnant women during mid and late gestation, and are considered physiologic. Striae associated with topical steroid usage usually occur at application sites, and may occur within weeks of initiation in susceptible persons. These may occur at any age. Striae associated with adrenal cortical hyperactivity occur at whatever age the process becomes clinically active. Early lesions may be edematous and irritable, but the central area quickly becomes depressed and is initially pink, bright red, or red-blue in color. Striae develop in parallel rows perpendicular to the direction of stretch or stress. Most lesions are 1 cm or less in width...

If adults are struck by an automobile or light truck, rather than a truck with a high front, a different pattern of injuries occurs because victims are impacted below the center of gravity. With non-braking or late-braking automobiles at very high speed, the pedestrian is picked up and thrown over the top of the car. Examination of the automobile reveals either scuff marks or dents on the bumper, as well as denting of the front of the hood in most instances. There may be dents on the roof or trunk of the car, when the individual is hurled over it. The authors have seen this pattern in individuals struck at high speeds, i.e., the high 60s and 70s mph range. In these cases, there is often mangling of the body with partial or complete amputation of a limb by the massive blunt trauma (Figure 9.15).
